Making a referral
Email David directly at [email protected], rather than contacting the host practices, as David (or his assistant Janet) make all the bookings. Please include in the referral request:
The clients will then be contacted directly to book a mutually convenient clinic venue, date and time for their first appointment. They will receive a patient information form to complete prior to their first consultation. A report will be sent directly to the referral vets and the owner after the first and any subsequent consultations detailing the diagnostic, treatment and management plans. All follow up appointments will be arranged directly with the clients and If you wish to speak to David, prior to the referral, phone 07929 863542 - please be prepared to leave a voice message and David will call you back or email any queries to [email protected] or dreogodfrey@gmail.com. N.B. For vets making a referral from within a host practice - please avoid using the internal practice messaging system. David is unlikely to pick up any referral via this route. |
Referral report service
To request a written referral report, please email to [email protected]
David will email the report to the referring veterinary surgeon with suggestions on how you might manage that patient in the future. |
